I have reviewed the Nursing Program Admissions Criteria and have met all requirements for admission into the nursing program at the NUR 110/120 (foundational level) and will meet with an academic advisor for clarity as needed.

I understand that my admission to the Nursing Program will be based on a thorough review of my application, transcripts and exam scores as well as space availability.

I have reviewed the Nursing Program Admissions Criteria and have met all requirements for entry specific to the LPN-RN Transition Program.
I further understand there are additional pre-requisites for LPN-RN Transition Program and will meet with an academic advisor for clarity as needed.

I understand that my admission to the Nursing Program will be based on a thorough review of my application, transcripts, exam scores and valid Pennsylvania license, as well as space availability.

I certify that I currently hold an active, valid LPN license in the state of Pennsylvania.

If I have attended another college or university, I understand that as part of my application, I must submit official transcripts from each post-secondary institution that I have previously attended either electronically to transcripts@ccac.edu or via postal mail to a Registration office. Further directions can be found on the Transfer Credit page.

I understand which documents are required for admission to the program and that I am responsible for insuring that they are submitted prior to the application deadline.

To apply to the Nursing Program a valid CCAC Student ID number is required.
